* You will need to bring your insurance card to each visit. You will not be seen without a card. If you have not been sent one from your insurance company, you can print a temporary card off your insurance company website.

* Payment of your co-pay is required at each visit. Cash, checks, and credit cards are accepted. Please see "Frequently Asked Questions" (FAQ) for an explanation of co-pays.

* A copy of your previous health records. You can contact your previous health care provider to have them faxed to us (fax number under "Contacting Us") or bring a copy in with you.

* If you are seeing us in referral for an abnormal test or pap result, please bring a copy of that test result in with you.
