WHAT IS A CO-PAY?

Your insurance company assigns a set amount for payment of services to your physician. The co-pay is what your insurance company has assigned as your responsibility to pay at the time of service as part of the total amount due. The insurance company deducts the co-pay amount from the payment paid to the physician. Insurance companies mandate that physicians collect the co-pay at the time of service and at each visit.

HOW MUCH WILL MY CO-PAY BE?

Every insurance plan is different. Most insurance cards list co-pay amounts on the front by physician or facility type. Most co-pays differ by primary or specialists physicians. We are specialists. Your insurance plan booklet will list co-pay amounts, and you can always call the customer service number on the back of the card for this information.

DO I NEED A REFERRAL?

Most insurance plans cover "Well Woman" care and have "open access" for problems. However, if you have any questions, contact your insurance carrier.

WILL ALL MY SERVICES BE COVERED BY MY INSURANCE?

Most exams and lab tests are covered for annual exams and problems. Certain screening tests, such as tests for sexually transmitted diseases without any symptoms, may not be covered by all plans. If you will be wanting such tests performed, you should check with your insurance carrier first.

DO I NEED TO BRING MY INSURANCE CARD?

Your insurance card is required at each visit. You can not be seen without it. If you have not been sent one from your insurance company, you can print a temporary card off their website, or call the customer service number listed in your booklet or on their website.
